Registration
or all camps will be processed on a first-come, first-served
basis.
Registration
form
Complete a separate registration form for each child. Photocopies
are acceptable, or print out another form. Read and complete the medical information
section and sign the registration form. Your signature is
required
for your child to attend camp.
Kids
Attending Together
If you want kids to attend camp together, put all the registrations in one envelope.
This is the only way we can place all the children in the sessions you request.
If you are faxing registrations send all the forms in one fax.
Payment
Payment must accompany your registration. We accept checks,
Visa, Mastercard, American Express, Discover. Checks
should be made payable to Oregon Zoo.
Registering
for 2 weeks of camp
You may register children entering grades 1-5 for a second
week of camp; however, the program and activities do not change
week to week. Use a separate registration form for the second
week.
Confirmation
Confirmation will be mailed in 5-7 days. Do not call to check
on your registration before that time. If there is a problem
with your registration, we will phone you.

Lists
Waiting
Lists are compiled for anyone not placed in any of
their choices. If an opening becomes available, we will contact
you by phone. Please make sure we have a daytime phone number
to each you.
Refunds
Refunds minus a 10% administrative fee, are only made if your cancellation is received before June 2nd.
After
June 2: 75% refunds are given if cancellations are made
at least 5 working days before the start of the camp session.
No partial refunds will be given for missed camp days.

Camp Registration Box at the reception desk in the
Zoo’s
main entrance.

Include a credit card number and signature. If you are
faxing
your
registration and have multiple children wanting to attend
together, all applicable registration forms must be faxed
together. The zoo is not able to confirm receipt of your
fax.
